I often design with the intent to enlarge in Acrobat using the poster option.  ie I just did a couple of single words and designed at 4 inches with the intent that they we enlarged 300% to 12 inches.

I originally specified the document at letter and when I enlarged it was going to take 15 pages.

I found an option when saving as PDF to use the exported object size.  Using that option it now takes 4 pages.

I have run into this when printing and found that I may have to select the option "Portrait" or "Landscape" even though it maybe on auto select of the orientation of the paper.  Must have something to do with how the designer exported it as you are describing.
